This form is for participants who need to cancel scheduled NDIS supports due to illness.
If you are unwell and unable to attend your scheduled support, please complete this form as soon as possible. Providing notice helps us notify your support workers and update your booking.
Please note that cancellations made with less than three working days' notice may still be charged to your NDIS plan in accordance with your NDIS Service Agreement and the NDIS Pricing Arrangements, as support workers may already have been scheduled and are entitled to payment.
